How to register

DURING THE COVID 20 HEALTH EMERGENCY, WE WILL ACCEPT PHOTOGRAPHED OR SCANNED PERMANENT REGISTRATION FORMS. PLEASE ENSURE YOU SEND THE COMPLETED GMS1 FORM AND THE New Patients History FORM ALONG WITH A SCANNED FORM OF PHOTOGRAPHIC ID

IF YOU NEED TO REGISTER AS A TEMPORARY RESIDENT DURING THE CORONAVIRUS 2020 OUTBREAK, please complete Temporary registration form, scan or photograph it and send it by email to the surgery to access our services on a temporary basis

 

Joining the practice is very simple. Just come along to either branch with the following documentation:

  • Registration form (GMS1): this can be collected at reception, or downloaded and completed in advance if you prefer.
  • Photo ID, such as your passport or driving license.
  • Proof of your address (a Council Tax or utility bill would be ideal).

You will be asked to complete a New Patient History form and can download this or collect it at reception. This provides us with important background medical history prior to your notes arriving at the surgery.

If you have a preference for a male or female doctor please make this known to the receptionist when you register.

You may be invited to a new patient appointment, particularly if you are on a lot of medication or have complex medical needs. Normally the GP will want to check and review your medication before we start to issue prescriptions.
