How to register

If you wish to join our practice, please come along to one of the surgeries and ask the reception staff for a registration form. You can download a registration form (GMS1) and complete this in advance if you prefer, just click on the icon below. When you register please bring some photo ID, such as your passport or driving license, together with proof of your address. A Council Tax or utility bill would be ideal. You may not have been asked for ID before, these requirements have been introduced quite recently to help prevent fraud.

If you have a preference for a male or female doctor please make this known to the receptionist when you register.

All new patients will need to complete a New Patient Medical History form. This provides us with important background medical history prior to your notes arriving at the surgery. You can download the form here and complete it before you attend.

We may need to see you for an initial appointment, particularly if you are on a lot of medication. Normally the GP will want to check and review your medication before we start to issue prescriptions.
